Failing to calibrate the closed vent system detection instrument before each use; failing to calibrate the monitors for the Corvus Oil Scrubber flow to the Corvus Oil Scrubber EBC1 and makeup flow rate for the No 9 Boiler Venturi Scrubber; failing to record a corrective action for an MIBK concentration greater than 1000 ppm; failing to maintain updated records for wastewater streams; failing to maintain documentation of and/or conduct weekly visual pump inspections; failing to maintain the HRVS pressure drops within permitted limits; failing to repair and/or confirm repair of leaking valves and pumps; failing to operate the CEMS on the No10 boiler within the specified accuracy tolerances; failing to submit semiannual reports; failing to submit opacity test results for the No 9 boiler |
