Failure to make a waste determination; failure to close satellite accumulation containers and label with the words "Hazardous Waste" and indication of the hazards of the contents; failure to manage satellite accumulation area wastes; failure to accumulate waste under the control of the operator of the process generating the waste; failure to label containers within a central accumulation area with the words "Hazardous Waste", an indication of the hazards of the contents, and accumulation start date; failure to conduct an annual review of initial training; failure to conduct and document weekly inspections of central accumulation areas; failure to include a map of the facility showing where hazardous wastes are generated, accumulated, and treated and routes for accessing these wastes; failure to make and document arrangements with local emergency response teams; failure to maintain and operate the facility to minimize the possibility of fire, explosion, or any unplanned release; failure to equip the CAA with a spill control kit, fire extinguisher, or decontamination equipment |
