| Requirements of Order |
Respondent shall have a certified wastewater operator collection monthly samples from 1) the Facility's effluent discharge structure, 2) upstream and 3) downstream of the discharge. At a minimum, the Respondent shall report Flow, TSS, 5-day BOD, Fecal, pH, and Dissolved Oxygen; w/in 30 days of the execution of this order, the Respondent shall provide the Division a detailed plan of abandonment of the Facility; by March 1 2019, the Respondent agrees to cease the unpermitted discharge and fully complete the abandonment of the Facility ; should the Respondent fail to meet any of the requirements or deadlines, Respondent agrees to pay the sum of $500.00 per month per condition until such time as the Respondent is in full compliance with conditions #2-4 above. |
