Cause of Order Failed to use reasonable methods to prevent and control pollution of the waters of the State; failed to maintain and operate efficiently all treatment or control facilities and related equipment installed or used to achieve compliance; failed to provide safety equipment, maintenance, and good housekeeping at the Facility and the collection system for the Facility, violating the approved Operation and Maintenance Manual
Requirements of Order W/in 30 days of the execution of this Order, submit to the Division a written, OSHA approved, Confined Space Entry procedure, and complete the repair of the Dean Street lift station and the Facility's dry well pump rooms' lights, fans, and sump pumps, in addition the Respondent will purchase a gas analyzer by this same deadline; w,in 30 days of execution of this Order, ensure all grease and trash is removed from the Beall Spring Street and Lamb Street lift stations; w/in 30 days have the auto pump switch repaired or replaced at the Railroad Street lift station; w/in 30 days have hourly pump run recorders installed at every lift station; w/in 30 days have all solid waste located outside the gate of the Facility removed and disposed of at a permitted landfill; w/in 90 days remove all trees and vegetation from the outer slope of the berm of both the aerated and anaerobic lagoons of the Facility, and shall repair the cement apron of the anaerobic lagoon; pay sum if failure to meet conditions
