Cause of Order |
Did not operate the Facility in such a manner as to conform and comply with all rules, regulations, orders and permits established under the Drinking Water Act; undertook substantial modification to the Facility by constructing and adding on to the Facility two unapproved wells without having first secured the Division's approval; failed to submit for the Facility's two unapproved wells engineering plans and specifications for Division review and approval prior to construction of the wells; failed to submit for the two unapproved wells at the Facility, the physical, chemical and radiological parameters (source approval) needed for approval of a groundwater source; operating the Facility without disinfection residuals in the system; operating without a well that meets installation standards; not keeping proper records; does not have on file a Sample Site Plan that meets the requirements of the Revised Total Coliform Rule |
Requirements of Order |
Comply with Act, Rules and Permit; immediately provide for continuous chlorination at the facility and maintain a detectable residual of free chlorine in all parts of the distribution system; submit Facility operational reports including monthly "Daily Operating Reports" in accordance with Drinking Water Rules and Permit; no later than 15 days from the execution date of this Order, submit to the Division a sample site plan for the Facility; submit to the Division for review and approval a Corrective Action Plan (CAP); upon receipt of CAP approval, implement the CAP; submit a final progress report from the licensed well driller or professional engineer outlining the Respondent's activities in implementing the CAP |
