Failed to comply with Permit groundwater effluent limitations; failed to comply with Permit which requires the Respondent develop a plan to ensure compliance with the groundwater parameter limitations established by the Permit whenever the Facility exceeds the established limitations, and to immediately implement the plan once approved; failed to submit monitoring information for the New Well for the months of July, August, September, October, November and December 2017 within the required timeframe; failed to submit monitoring information for the New Well for the months of January, February, March, April, May, June, and July 2018 in the required timeframe; failed to maintain the Facility as a no-discharge system as required by the Permit and failed to submit the required CAP |
