Cause of Order	Dawnville WPCP failed to submit a DDR by January 1, 2015 as required by NPDES Permit; Dawinville WPCP failed to submit plans and specifications for modifications to the facility by May 1, 2015; Dawnville WPCP to begin construction of any modifications to the facility by October 1, 2015; Dawnville WPCP failed to submit a report to the Division regarding the progress made towards completing modifications to the facility by January 1, 2016; Dawnville WPCP failed to attain compliance with the TRC limitation by July 1, 2016; Dawnville WPCP exceeded the Daily Maximum Limit for TRC during months of August, September, October, November, December 2016, January, February, March, April, May, June, August, September, October, November, December 2017, January, February, March, April 2018; Varnell WPCP failed to submit a DDR by January 1, 2015; Varnell WPCP failed to submit plans and specifications for modifications to the facility by May 1, 2015; Varnell WPCP failed to begin construction of any modifications to the facility by October 1, 2015; Varnell WPCP failed to submit a report to the Division regarding the progress made towards completing modifications to the facility by January 1, 2016; Varnell WPCP failed to attain compliance with the TRC limitation by July 1, 2016; Varnell WPCP exceeded the Daily Maximum Limits for TRC during the months of August, September, October, November and December 2016, January, February, March, April, May, June, August, September, October, November and December 2017, January, February, March and April 2018; Westside ES WPCP failed to submit a DDR by July 1, 2015; Westside ES WPCP failed to submit plans and specification for modifications to the facility by November 1, 2015; Westside ES WPCP failed to begin construction of any modifications to the facility by April 1, 2016; Westside ES WPCP failed to submit a report to the Division regarding the progress made towards completing modifications to the facility by July 1, 2016; Westside ES WPCP failed to attain compliance with the TRC limitation by January 1, 2017; Westside ES WPCP exceeded the Daily Maximum Limits for TRC during the months of January, February, March April, May, June, July, August, September, October, November, December 2017, January, February, March and April 2018; Westside MS WPCP failed to submit a DDR by October 1, 2015; Westside MS WPCP failed to submit plans and specifications for modifications to the facility by February 1, 2016; Westside MS WPCP failed to begin construction of any modifications to the facility by July 1, 2016; Westside MS WPCP failed to submit a report to the Division regarding the progress made towards completing modifications to the facility by October 1, 2016; Westside MS WPCP failed to attain compliance with the TRC limitation by April 1, 2017; Westside MS WPCP exceeded the Daily Maximum Limits for TRC during the months of April, May, June, August, September, October, November, December 2017, January, February, March, April 2018
Requirements of Order	W/in 30 days of the execution date of this Order, submit for EPD review and approval a Corrective Action Plan and an associated timeline for completion that will be implemented to prevent future parameter violations for TRC at the Facilities and when approved by the Division, will become part of this Order and must be executed per the timeline for completion in the CAP; if the respondent implements the approved CAP, then any expenditures made directly by the Respondent toward completing the CAP may be used to offset, in partial or in full, the stipulated penalty
