Cause of Order Respondent failed to: provide the overfill prevention system for an existing tank, operate and maintain corrosion protection system continuously, inspect impressed current cathodic protection system every 60 days, repair UST system in accordance with industry codes and standards, monitor tanks at least every 30 days, comply with financial responsibility requirements, have a certified Class C Operator
Requirements of Order Within 30 days from the execution of this Administrative Order: submit documentation to the Division verifying the type and proper operation of the overfill prevention system; have a corrosion expert repair the cathodic protection (CP) system and conduct a CP test; create a rectifier log and submit the log with the initial rectifier reading to the Division; remove and properly dispose of any fuel, water, and debris from spill buckets, sumps and submit documentation of removal to the Division; conduct tank tightness testing; initiate monthly ATG monitoring, and submit the tightness test results and the initial ATG monitoring report to the Division; submit a copy of one invoice per quarter of fuel delivery invoices for the last 3 years, or submit a letter from the facility's jobber to verify environmental assurance fees are being paid into the GUST Trust Fund, or, if the facility is not a participant in the GUST Trust Fund, provide documentation of a financial assurance mechanism that ensures compliance with financial responsibility requirements; certify all applicable employees and submit copies of their Operator C training certificates to the Division; effective immediately, the UST Registration Certificate for the Respondent's UST facility is suspended and fuel delivery to the Respondent's UST facility is prohibited
