Cause of Order Failure to obtain a permit for the disposal and storage of hazardous waste; failure to inspect the areas where hazardous waste containers were stored; failure to label approximately six 5-gallon buckets of hazardous waste in the Central Accumulation Area with an accumulation start date; failure to close the container of spent fluorescent lamps stored in the maintenance area; failure to label or mark each lamp or a container or package in which such lamps are contained clearly with one of the following phrases: "Universal Waste-Lamp(s)" or "Waste Lamp(s)" or "Used Lamp(s)"; failure to document the amount of time the spent fluorescent lamps were accumulated; failure to label one 5-gallon bucket of used oil in the maintenance shop area with the words "Used Oil"
Requirements of Order Immediately halt the on-site disposal of hazardous waste; immediately make arrangements to properly dispose of the hazardous waste stored in excess of 180 days; w/in 30 days of the execution date of this Consent Order, submit for the Division's approval plans to ensure the hazardous waste at the facility is managed properly in accordance with the Hazardous Waste Rules; w/in 90 days of the execution date of this Consent Order, submit documentation to the Division that the Facility is operating in compliance with the plans described in Condition 4
