Cause of Order Exceeded the discharge limits at the Facility for Ammonia in April, May, June, July, August, and September 2018; for 5-day Biochemical Oxygen Demand (BOD) in April, May, June, July, August, September, November and December 2018; and January, February and March 2019; for Fecal Coliform in May, June and December 2018; and for not meeting the minimum Dissolved Oxygen level in March, April, June and July 2018; failed to ensure compliance with the terms and conditions of the Permit
Requirements of Order W/in 15 days of the execution date of this Order, the Respondent shall provide the Division with a letter of commitment stating the City shall provide the means to maintain the Facility in accordance with the maintenance schedule found in the Pearson Water Pollution Control Plant (WPCP) Operations and Maintenance (O&M) manual revised January 2005; w/in 30 days of the execution date of this Order, the Respondent shall provide the Division with a written report detailing the findings of the manhole assessment for illicit dumping activity; demonstrate the performance of the Facility for 6 months following the execution date of this Order, during the seasonal time period when significant effluent violations were experienced in the previous year ; implement the CAP upon receipt of the Division's approval, and within 10 days provide written confirmation that the CAP is being implemented as approved; submit to the Division a final progress report from the professional engineer outlining the Respondent's activities in implementing the CAP ; Respondent shall obtain approval from the Division for any engineering reports, plans and specifications related to modification of the sewerage system
