Unlawfully engaged in solid waste handling; failed to cover disposed solid waste with 6 inches of clean earth cover; failed to maintain vector control; failed to maintain a sufficient supply of earth cover for daily cover and stockpiled soil for fire protection within 200 feet of the working face of landfill; failed to maintain effective erosion and sedimentation control to prevent slope erosion; failed to timely submit a properly revised D&O in conjunction with the 5 year permit review submission; failed to apply the monthly cover at the C&D landfill; failed to submit separate Quarterly Tonnage Reports detailing total amount of solid waste disposed; failed to provide Notice of Final Closure to Director within 30 days of receiving the final load of waste and complete closure activities |
| Requirements of Order |
Agree to comply with the Solid Waste Act, Rules and MSW and C&D Landfill Solid Waste Handling Permits; uniformly cover all disposed solid waste at MSW Landfill with 6 inches of clean earth at the end of each working day; litter fence shall be erected downwind of the working face of the MSW Landfill and daily inspection and removal of all litter shall be instituted; begin applying monthly cover to the working face of the C&D landfill; provide its latest survey for the MSW landfill and C&D landfill; inspect all erosion and sedimentation structures monthly or after any storm event; submit a corrective action plan prepared by a professional engineer; submit a final report certified by a professional engineer detailing compliance with all conditions of this Order |
