Violated EPD-SW-2848 for the following reasons: facility continues to vertically expand the landfill without approval, facility had not covered exposed/flagging waste, immediate cover was not applied corrected and it was not being maintained resulting in exposed waste, vegetation in and around Sediment Pond A was not being maintained, disturbed areas in Borrow Areas A and C have not been stabilized, sediment is being discharged from the northwesters corner of Borrow Area A, several leachate outbreaks, erosion and sedimentation control structures are not being maintained, some of the environmental monitoring wells were overgrown and unlocked, dust control was not being implemented, scale attendant was allowing waste not defined as construction/demolition material, landfill was not graded properly in many areas, bare areas throughout the landfill have not been vegetated, scrap tires have exceeded the capacity of the storage container and not being stored in a manner that pose fire and vector hazards, cleanout level on the sediment markers cannot be identified, sediment ponds A and B need cleaning |
