Cause of Order Runoff from the wood chip fuel pile that passed through the settling basins and discharged to an unnamed tributary to Indian Creek produced significant water quality changes that were potentially lethal to aquatic life and resulted in a fish kill; failure to maintain and operate Basic C as efficiently as possible by allowing discharge through the leaking outfall structure; submittal of inaccurate information in the DMR for October for Outfall 001; submittal of DMRs after the due date; effluent limit violations at Outfall 001 and 002; failure to monitor BOD at Outfall 002
Requirements of Order Submit the Facility's standard operating procedure for monitoring wood pile runoff and monitoring and adjusting pH at Outfall 001 and 002; submit a corrective action plan with an implementation schedule to address alleged effluent limit violations at Outfall 001 and/or 002; submit all analytical results from sampling following the October 5, 2019 discharge event
