complete the required Lead and Copper sampling at the Arlington Terrace Water System; issue Public Notification to the customers of the Arlington Terrace water system to satisfy the Lead and Copper failure to monitor violation; issue Public Notification to all applicable customers of the CJT Group, LLC water systems to satisfy the various Groundwater Rule and Revised Total Coliform Rule failure to monitor violations; complete all required IOC, VOC, DBP, and Nitrate sampling at all 5 of the CJT Group, LLC water systems; issue Public Notification to all applicable customers of the CJT Group, LLC water systems to satify the chemical failure to monitor violations; prepare the 2016, 2017, 2018, and 2019 CCRs for all 5 of the CJT Group, LLC water systems, prepare public notification for the CCRs to the customers served by all 5 of the CJT Group, LLC water systems, and submit a copy of all CCRs and CCR Certification forms to EPD; submit all necessary documentation to this office to obtain approval to upgrade the storage tank at the Arlington Terrace water system; install and place into service the approved equipment at the Arlington Terrace water system; obtain Performance Bonds or Letters of Credit for each water system |
