| Cause of Order |
Failing to submit a renewal NOI to continue required 2018 Permit coverage and maintain subsequent compliance at the Site; failing to install and/or maintain BMPs at the site; as a result of failing to install and/or maintain BMPs, sediment was discharged from the Site and deposited into Waters of the State on 2 separate occasions; as a result of failing to install and/or maintain BMPs, turbid storm water discharged from the site; failing to ensure the ESPCP was amended as required within 7 calendar days after the completion of each BMP inspection; failing to ensure the certification of the ESPCP after the Respondent installed hydraulic BMP changes; failing to conduct storm water monitoring during the 3 consecutive qualifying rain events; failing to document all incidences of noncompliance in the weekly and post rain BMP inspection reports; failing to submit a violation summary self-report to the Division for sediment discharges from the site that entered Waters of the State; failing to submit a violation summary self-report to the Division for any and all known violations occurring at the Site |
